We offer preventative and primary healthcare services through our fixed site in North Seattle and our Mobile Medical Clinic (MMC) locations.

Our patients have access to full medical exams, resources to obtain low to no cost prescription drugs, laboratory tests, imaging services, intensive nursing case management and referrals to medical specialists. Additionally, we have expanded in offering several forms of specialty care at some of our sites including: individualized diabetic education, physical therapy, ophthalmology, gynecology and chiropractic care.

Patients can be seen at no cost, but we do ask for a $10 Suggested Co-Pay per appointment.

We are not a Walk –In or Urgent Care clinic and cannot make arrangements for those appointment types. If this is an urgent issue, please go to your local urgent care clinic, seek care at an emergency room, or call 911 immediately. If this is a mental health crisis, please call 988 to speak with someone at Lifeline Crisis Center.

Lahai Health cannot provide services for L&I claims.

For New Patients

Prior to your appointment, please fill out these 2 parts of electronic forms and submit to Lahai Health. Computer tablets will be available at your appointment if you are unable to complete forms before your appointment. Please come 30 minutes early to your appointment, bring government issued ID (ex. license or passport), proof of income (3 consecutive months of pay stubs), and bottles of any medications you are taking. If patient has no source of income, please fill out form Explanation of Patient Financial Standing, which can be found below.

For Returning Patients

Patients must fill out the registration form every year. Returning patients with NO CHANGE in medical history can skip the medical history part (2) and just fill out the Demographics portion (Part1). OF course, if there have been changes in the past year, then you are to fill out the entire form (Part 1 and 2). If this is your first visit of the year, please fill out the electronic form prior to your appointment. Also bring proof of income such as tax documents from previous year(preferred) or two consecutive months of financial documents (ex. bank statement, pay stubs). If this is January or February, you can upload or bring proof of income from the last six months.
